Root Cause Analysis
Non-Negotiable Requirement: Carriers require the exact phrase "Msg & data rates may apply" (or "Msg rates may apply") to be present in the first message.
The Fix
Insert the phrase verbatim. Do not get creative ("Standard rates apply" is sometimes rejected by stricter validators).
